Sciatica Treatment in Frisco, TX

Shooting pain from the low back into the leg is a nerve problem, and treating the muscle it runs through will never solve it.

The sciatic nerve is the largest nerve in your body, roughly the diameter of your thumb where it exits the pelvis. It is formed from nerve roots in the lower lumbar spine and sacrum, runs through the buttock, and travels down the back of the leg to the foot. When it gets compressed or irritated anywhere along that path, you feel it along the whole path.

That is why sciatica is so distinctive and so miserable. It is not a dull local ache. It is electric, shooting, and it goes somewhere, down the back of the thigh, past the knee, sometimes all the way into specific toes.

Symptoms we hear about

  • Sharp, burning, or electric pain from the low back into one leg
  • Numbness or pins-and-needles in the leg, foot, or toes
  • Weakness in the leg or a foot that feels like it is dragging

Where exactly is the nerve being pinched?

Sciatica is a description of symptoms, not a cause, and effective treatment depends entirely on identifying the source. A disc herniation at L5-S1 and piriformis compression in the buttock produce similar-sounding pain and require genuinely different treatment.

Our exam maps which nerve root is involved by testing sensation, reflexes, and strength in the specific patterns each root controls. That tells us the level. Orthopedic testing and imaging then tell us the mechanism.

Take the pressure off first

When a disc is the cause, which it is in the majority of true sciatica, non-surgical spinal decompression is our primary tool. Cycling gentle traction on the involved segment creates negative pressure inside the disc, encouraging the herniated material to retract away from the nerve root and rehydrating tissue that has been degenerating.

We pair that with MLS cold laser directly over the nerve pathway to reduce the inflammation that is a large part of why the nerve hurts, and with adjustments to restore motion above and below the involved level.

When it is the piriformis, not the disc

In a meaningful minority of cases the nerve is being compressed in the buttock by the piriformis muscle rather than in the spine. These patients respond very differently, soft tissue work, specific stretching, and correcting the hip mechanics that caused the muscle to overwork are the treatment, and decompression does very little.

Sorting this out correctly at the start is the difference between resolving in six weeks and spending six months on the wrong protocol.

Will sciatica go away on its own?

Some cases do resolve over weeks to months. But recurrence is very common because the underlying disc or joint problem is still there, and each recurrence tends to be more stubborn. Treating it addresses the mechanism rather than waiting out the symptom.

Should I stretch a sciatic nerve?

Carefully, and it depends on the cause. Aggressively stretching a nerve that is already inflamed and compressed frequently makes things worse. Piriformis-driven sciatica responds well to specific stretching; disc-driven sciatica usually does not. We will show you which applies to you.

How long does sciatica treatment take?

Most patients notice meaningful change within two to three weeks. Disc-related cases on a decompression protocol typically run six to eight weeks. Long-standing sciatica with significant degenerative change takes longer, and we will tell you that up front rather than after you have started.

Can I avoid surgery?

Most people with sciatica never need it. Conservative care resolves the large majority of cases, and surgery is generally reserved for progressive neurological deficit or failure of appropriate conservative treatment.
